Implementation Research on CPHC to transform Mysuru city into a Healthy City h (2017)
We developed a comprehensive continuum of care model to assess and enhance and assess care for Non-Communicable Diseases (NCDs) and improve health outcomes in urban settings. We conducted a mixed-methods study to estimate denominators for those with disease and at risk, to understand the distribution of risk factors, treatment adherence and out of pocket expenses; to assess readiness of public and private facilities, and to understand practices, myths and perceptions in the community The study resulted in the development of tools and methodology to gather evidence in designing comprehensive NCD programmes in low and middle income settings.
Our NCD model was presented along with the Government of Karnataka to NITI Aayog in 2018. Our implementation research design and tools were adopted by the Indian Council of Medical Research for the NCD National Task Force Project among tribal populations in six states. Based on our NCD experience, we are moving to transform Mysuru to a Healthy City through a through a Whole-of-Society and Whole-of-Government Approach.

Identification of Health and Wellness Centres and Polyclinics in urban areas through mapping
With the support of the Mysuru City Corporation, we developed and pilot-tested a mapping exercise to identify existing Urban Primary Health Centres (UPHCs) to be designated as Urban Health and Wellness Centress and Polyclinics to maximize the coverage of vulnerable populations and best utilize existing resources.
The process involved identifying boundaries and populations served by UPHCs, and consulting Census and administrative documents to understand the proportion of slum populations served by UPHCs. To select UPHCs to be identified as polyclinics, we assessed the readiness of the facility based on their infrastructure, including equipment to run routine tests and laboratory facilities.
This scientific approach to mapping enabled the identification of strategically located and equipped UPHCs to be converted into UHWCs and Polyclinics in the nine administrative zones of Mysuru. It has the potential to be adapted in metro cities and small towns across the country, in alignment with the recommendations of the 15th Finance Commission and the principles of the PM-ABHIM and Ayushman Bharat scheme.

Implementing Convergence and Decentralization through the Graama Panchayath Arogya Amrutha Abhiyaana
The Graama Panchayath Arogya Amrutha Abhiyaana (GPAAA) is a first-of-its-kind convergence initiative in the country launched by the Government of Karnataka, which hopes to usher in new synergy among the Gram Panchayat (GP), communities and the health facilities in rural areas of the state. We have been working with the Departments of Rural Development and Panchayat Raj (RDPR), Health and Family Welfare (HFW), and Women & Child Development (WCD) osince August 2021, to involve GPs more proactively in addressing public health and social issues including diabetes, hypertension, anaemia, TB, nutrition, child marriage, mental health and menstrual hygiene. We provided a health management kit of point-of-care testing devices to GPs in 14 districts to enable health screening through camps in the communities and workplaces for people who find it difficult to reach health facilities.
In light of the increased screening and testing for TB and non-communicable diseases (NCDs), the Government of Karnataka made a provision in its annual budget in 2022 to scale GPAAA to over 5900 Panchayats in all 31 districts in the state. We have supported the screening of over 28 lakh people for TB, 13 lakh people for NCDs and 1.4 lakh women for anaemia.
